Definition of Purge
Use of vomiting, laxatives, diuretics, enemas to eliminate calories
Definition of Binge
Eating a large amount of food within a discrete period
Eating is out of control during the episode
Anorexia Nervosa Diagnostic Criteria
- Restriction of food leading to <18.5 BMI
- Intense fear of weight gain
- Body image disturbance
Subtypes of Anorexia Nervosa
- Binge-eating/purging Type (only needs one)
- Restricting Type: does not binge/purge but weight loss is through dieting, fasting, +/- exercise
Anorexia Nervosa: Associated Features
- Amenorrhea, loss of sex drive, constipation
- Hair loss
- Dry skin, brittle nails
- Low HR, low BP, and low temp
- LANUGO
Treament of Anorexia Nervosa
In hospitalization
Usually referred by family-pt does not believe it is a problem
1. Slow feeding in hospital (weight gain)
2. Behavioral principles (operant conditioning) to encourage weight gain
3. Supervision of eating to thwart behaviors to lose weight
What is the treatment for ALL Eating Disorders?
CBT
Bulimea Nervosa Diagnostic Criteria
- Recurrent binge eating
- Inappropriate compensatory behavior (purging, fasting, excessive exercise)
- Once a week for at least 3 months
- Self evaluation is unduly influenced by body shape and weight
Associated Features with Bulimia Nervosa
- Electrolyte Imbalances
- Arrhythmias
- Consequences of excessive vomitting
What are the consequences of excessive vomitting?
- Tooth Decay
- Salivary gland inflammation (chipmunk cheeks)
- Russell’s sign (knuckles callused)
- Tearing of tissue in stomach and throat
What is the only med that is FDA approved for Bulimia Nervosa?
Fluoxetine
Binge-Eating Disorder Diagnostic Criteria
Binge eating but then No inappropriate compensatory behavior
Must be more than 1 x wk for > 3 months
Are all obese people diagnosed w/ binge-eating disorder?
NO-there is greater distress/functional impairement in binge eating disorder
Treatment of Binge-Eating Disorder
- CBT
- Antidepressants
Unspecified Eating Disorder Criteria
Do not meet criteria for any other disorder-even if the syptoms are concerning
