What are the clinical features of wax build up?
gradual hearing loss, ear discomfort, recent attempts to clean ear
itchiness, tinnitus and dizziness occur infrequently and this affects inner ear
When to refer a pt with ear wax build up?
- Dizziness, tinnitus - inner ear
- Unusual discharge from the ear canal
- Persistent pain from within the ear
- Fever, general malaise in children
What are side effects that can occur?
local irritation and increase deafness at first but will pass
What is general advice you would give to patients ?
do not use cotton buds, hairgrips, matchsticks, pens or any other utensils
What are oil-based products that can be used? What should patients be warned about when using these products?
- Cerumol Ear drops (peanut oil) - 5 drops into the affected ear two to three times a day for up to 3 days
- Cerumol Olive oil drops - two to three drops BD for up to 7 days
- Earex - peanut oil, almond oil etc- dour drops BD for up to 4 days

What are peroxide-based products?
Exterol, earex advance, otex
react with catalyse enzyme to release oxygen which mechanically breaks wax up
five drops once or twice daily for at least 3-4 days - effervescence may occur
What are water-based products?
sodium bicarbonate
2-3 times a day for up to 3 days
what other tx options are available?
Docusate - facilitates penetration of water
Glycerin-based products - acts as softeners
