What are the clinical features of wax build up?

A

gradual hearing loss, ear discomfort, recent attempts to clean ear

itchiness, tinnitus and dizziness occur infrequently and this affects inner ear

When to refer a pt with ear wax build up?

A
  1. Dizziness, tinnitus - inner ear
  2. Unusual discharge from the ear canal
  3. Persistent pain from within the ear
  4. Fever, general malaise in children

What are side effects that can occur?

A

local irritation and increase deafness at first but will pass

What is general advice you would give to patients ?

A

do not use cotton buds, hairgrips, matchsticks, pens or any other utensils

What are oil-based products that can be used? What should patients be warned about when using these products?

A
  1. Cerumol Ear drops (peanut oil) - 5 drops into the affected ear two to three times a day for up to 3 days
  2. Cerumol Olive oil drops - two to three drops BD for up to 7 days
  3. Earex - peanut oil, almond oil etc- dour drops BD for up to 4 days

What are peroxide-based products?

A

Exterol, earex advance, otex

react with catalyse enzyme to release oxygen which mechanically breaks wax up
five drops once or twice daily for at least 3-4 days - effervescence may occur

What are water-based products?

A

sodium bicarbonate

2-3 times a day for up to 3 days

Docusate - facilitates penetration of water
