A man of 80 has a long history of heart failure secondary to ischaemic heart disease. He presents with increasing breathlessness. He is unable to lie flat and any exertion induces dyspnoea. He has markedly swollen ankles, a raised JVP, a gallop rhythm, a persisting tachycardia and a blood pressure of 100/60 mmHg. He is currently taking furosemide and an ACEI in adequate doses and is warfarinised.
What other treatments could be tried?
Replace frusemide with bumetanide BB MRA ? Salcubitril valsartan Digoxin If diuretic resistance Add thiazide diuretic to loop diuretic
