Oppositional Defiant Disorder
A: A pattern of angry/irritable mood, argumentative/defiant behavior, or vindictiveness lasting at least 6 mos as evidenced by at least 4 sx from any of the following categories & exhibited during interaction w/ at least one individual who is not a sibling:
Angry/Irritable Mood
1. Often loses temper
2. Is often touchy or easily annoyed
3. Is often angry & resentful
Argumentative/Defiant Behavior
4. Often argues w/ authority figures or, for children & adolescents, w/ adults
5. Often actively defies or refuses to comply w/ requests from authority figures or w/ rules
6. Often deliberately annoys others
7. Often blames others for their mistakes or misbehavior
Vindictiveness
8. Has been spiteful or vindictive at twice within the past 6 mos
Note: for children younger than 5 years the behavior should occur on most days for at least 6 mos. over 5 years older behavior should occur at least once per week for at least 6 mos
B: The disturbance is associated with distress in the individual or others in their immediate social context
C: The behaviors don’t occur exclusively during the course of a psychotic, substance use, depressive or bipolar disorder. Criteria not met for disruptive mood dysregulation disorder
Specify current severity:
Mild- sx are confined to only one setting
Moderate-some sx are present in at least two setting
Severe-some sx are present in three or more setting
Intermittent Explosive Disorder
A: Recurrent behavioral outbursts representing a failure to control aggressive impulses as manifested by either of the following:
1. Verbal or physical aggression toward property, animals, or other individuals, occurring twice weekly for about 3 mos. The physical aggression does not result in damage or destruction of property or result in physical injury to animals or people
2. Three behavioral outbursts involving damage or destruction of property and/or physical assault involving physical injury against animals or other period in a 12 mo period
B: The magnitude of aggressiveness expressed is out of proportion to the stressor
C: The recurrent outbursts are not premeditated & not committed to achieve a tangible objective (money, power, etc)
D: The recurrent outbursts cause distress or impairment
E: Chronological age is at least 6 years old
F: The recurrent outbursts are not better explained by another mental disorder
Note: can be made in addition to ADHD, conduct disorder, ODD or autism when recurrent outbursts are in excess of those usually seen in these disorders & warrant independent clinical attention
Conduct Disorder
A: A repetitive & persistent pattern of behavior in which the basic rights of others or societal norms or rules are violated, manifested by the presence of at least 3 of the following 15 criteria in the past 12 mos , at least one criterion in the past 6 mos:
Aggression to People & Animals
1. Often bullies, threatens or intimidates others
2. Often initiates physical fights
3. Has used a weapon that can cause serious physical harm
4. Has been physically cruel to people
5. Has been physically cruel to animals
6. Has stolen while confronting victims
7. Has forced someone into sexual activity
Destruction of Property
8. Has deliberately engaged in fire setting w/ the intention of causing serious damage
9. Has deliberately destroyed other’s property
Deceitfulness or Theft
10. Has broken into house, building, or car
11. Often lies to obtain goods, favors or avoid obligations
12. Has stolen items of nontrivial value w/o confronting a victim (shoplifting , forgery, etc)
Serious Violations of Rules
13. Often stays out at night despite parental rules, beginning before 13
14. Has run away overnight at least twice while living with guardians or once w/o returning for a lengthy period
15. Is often truant from school, beginning before 13
B: Disturbance is behavior causes impairment
C: If 18 or older, criteria not met for antisocial personality
Conduct Disorder Specifiers
Specify Whether:
Childhood onset type-show at least one sx of conduct disorder prior to age 10
Adolescent onset type: show no sx prior to age 10
Unspecified onset: criteria for dx is met, but there isn’t enough info available to determine age of onset
Specify If:
W/ limited prosocial emotions- to qualify two of the following characteristics persist of over 12 mos & in multiple relationships & settings:
Lack of remorse or guilt
Callous-lack of empathy
Unconcerned about performance
Shallow or deficient affect
Specify Current Severity:
Mild-few, if any conduct problems in excess of those required & conduct problems cause minor harm
Moderate-the number of conduct problems & the effect on others are intermediate
Severe-many conduct problems in excess of those required or conduct problems cause considerable damage to others
Pyromania
A: Deliberate & purposeful fire setting on more than one occasion
B: Tension or affective arousal before the act
C: Fascination with, interest in, curiosity about, or attraction to fire & its situational contexts
D: Pleasure, gratification, or relief when setting fires or when witnessing or participating in their attention
E: The fire setting is not done for monetary gain, as an expression of sociopolitical ideology, to conceal criminal activity, to express angry or vengeance, to improve one’s living circumstances, in response to a delusion or hallucination, or as a result of impaired judgement
F: The fire setting is not better explained by conduct disorder, a manic episode, or antisocial personality disorder
Kleptomania
A: Recurrent failure to resist impulses to steal objects that are not needed for personal use or for their monetary value
B: Increasing sense of tension immediately before committing the theft
C: Pleasure, gratification or relief at the time of committing the theft
D: The stealing is not committed to express anger or vengeance & is not in response to a delusion or a hallucination
E: The stealing is not better explained by conduct disorder, a manic episode, or antisocial personality disorder
