Hallmarks for DIC
excess clotting and bleeding
formation of microvascular thrombi in various organs due to multiple organ dysfunction syndrome
consumptions and subsequent exhaustion of coagulation proteins and platelets may induce severe bleeding
DIC pathophysiology
complex systemic thrombohemorrhagic disorder involving:
the generation of intravascular fibrin and the consumption of of procoagulants and platelets
the clinical condition is characterized by intravascular coagulation and hemorrhage
always caused by some underlying condition
what is DIC triggered by
sepsis trauma malignancy obstrectrical complications vascular disorders reaction to toxin: snake venom, drugs immunologic disorders: severe allergic rxn, transplant rxn
signas and symptoms of DIC
profuse bleeding from many items- needle puncture site, mucosa, surgical incision or laceration bleeding, multiple bruises or petechiae
thrombosis
associated conditions: acute tubular necrosis, adult respiratory distress syndrome, purport fulminans
how does DIC present similar to sepsis?
tachycardia hypotension fever hypoxemis tachypnea

supportive care:
ABC management
cardipulmonary support
treat underlying disorder
transfuse blood products as needed
heparin sometimes used
only treatment is to treat the cause
complications of DIC
acute kidney injurt change in mental status respiratory dysfunction liver dysfunction life threatening thrombosis and hemorrhage cardiac tamponade hemothorax intercerebral hematoma gangrene and loss of digits shock death
prognosis of DIC
depends on underlying disorder, but mortality is generally very high. mortality in septic patients is 30-50%
