Nephrogenic Diabetes Insipidus
kidney ADH receptors don’t work/unresponsive
Stimulation of ADH
Increased ECF osmolality stimulates osmoreceptors in the hypothalamus
Hypotension
Volume contraction leding to increased ECF osmolality
Actions of ADH/Vasopressin
Stimulates reabsorption of water in distal tubule and collecting ducts
Leads to increased water in ECF, deceased ECF osmolality
Use of ADH/Vasopressin
IM/IV over an hour in LRS or 5% dextrose for dx of DI
Tx of CDI?
Desmopressin
What is Desmopressin and how does it work?
Vasocontrictor activity?
How is it given?
synthetic ADH analog that is more potent and has a longer half life than natural ADH
Minimal vasoconstricotr action (more selective on V2 than V1)
Given parenterally or nose drops in conjuctival sac.
Drug of choice to tx Nephrogenic diabetes insipidus
Thiazide diuretic
Central Diabetes Insipidus
caused by ADH deficiency
