Cause: 
Dementia
Electolyte disorders
Lung, liver, heart, kidney, brain
Infection
Rx drugs (MC)

Unfamiliar environment
Metabolic

A
SS: 
clouding of consciousness
difficulty maintaining or shifting attention
disorientation
illusions
hallucinations
fluctuating levels of consciousness
Eval:
cognitive testing: MMSE and CAM
H&P
med review
lab tests
search for occult infection
further search when no cause found
Txt:
Treat underlying cause
create supportive and familiar environment
antipsychotic/benzodiazepine
