Do ID cards need to be returned when leaving employment?
Yes.
Members do not need to carry their ID card all the time while conducting fire department business, true or false?

What do members have to wear when getting their photo ID picture taken?
Dress shirt, collar device (officers), and a tie.
Where do newly appointed members receive their ID cards?
At the fire academy.
Who takes a members photo when they are bring promoted?
Personnel Office. VCU will take the picture and produce a new one.
Does a member have to turn in their old ID card when receiving their new one after being promoted?
Yes. They will receive the new one after the swearing in ceremony. They will also receive a new badge and ensignia from the personnel department.
What should a member do if they lose their ID card or it’s rendered unusable?
Notify their company officer or superior giving all pertinent facts. Members must contact the local police district to file a report of the missing ID card.
Can retiring members retain their ID card?
Yes. Members must inform the personnel office in writing. The member will turn in their ACTIVE card to personnel. Personnel will tell the fire prevention division to prepare a RETIRED ID card. The member then must make an appointment with the fire prevention LT to sign for and pick up the ID.
What does the company officer do when notified that a member lost their ID card?
They will prepare a memo requesting issuance of a new ID card. The memo will give full details surrounding the loss or destruction, the police district contacted, the police officer contacted, date & time of the report and sent to deputy commissioner of operations for approval.
Dies the member have to pay for a new ID card?
Yes. The member must include a check payable to the city of Philadelphia for $25 with the memo.
What happens once the request is approved?
A copy of the approved memo will be returned to the company. The member will then contact the fire prevention LT to pick up.
Is a new photo required when replacing a lost ID card?
Only if it has been over 5 years since the last picture or the members appearance has changed significantly.
What happens when the member picks up the new ID card?
The LT will have the member sign the approved memo which will be forwarded to the personnel office to be filed in the member’s personnel folder.
