Cushing's (Hypercortisolism) Flashcards
1
Q
Causes of hypercortisolism
A
- Cushing’s syndrome/disease
- Iatrogenic
- Stress
- Alcohol
- Caffeine
- Lack of sleep
2
Q
Cushing’s syndrome
A
Hypersecretion of cortisol due to adrenal pathology
3
Q
Cushing’s disease (more common)
A
Hypersecretion of cortisol due to ACTH-secreting pituitary adenoma
4
Q
Cushing’s syndrome causes
A
ACTH-independent
- Tumour (adenoma, carcinoma) of adrenal cortex
- Corticosteriod therapy
- Bilateral adrenal hyperplasia
5
Q
Cushing’s disease causes
A
ACTH-dependent
- Tumour (adenoma, carcinoma) of pituitary gland
- Ectopic ACTH secretion e.g. lung carcinoid
6
Q
Hypercortisolism symptoms
A
- Proximal muscle wasting
- Skin thinning
- Facial mooning
- Central obesity (stretch marks)
- Supraclavicular fat pads
- Easy bruising
7
Q
Hypercortisolism complications
A
- Infection
- Osteoporosis
- Depression
- Hypertension
- Diabetes mellitus (insulin antagonism)
8
Q
Hypercortisolism investigations
A
- Confirm
- 24-hour urinary free cortisol
- Dexamethasone suppression testing - ACTH testing
- Raised: pituitary cause
- Suppressed: adenoma cause - Localisation (imaging)
- MRI
- CT
9
Q
Hypercortisolism management
A
- Surgical resection of adenoma
- Somatostatins
- Radiotherapy
