Critical care nutritioin Flashcards
causes of mucosal atrophy
starvation, bowel rest ( NPO), parenteral nutrition, not enough glutamine
If someone is hemodynamically unstable what can happen to the GI system
decreased oxygen supple can cause ischemia resulting in death to the villi
When should EN be held in the ICU
during hypoperfusion to the gut, decreased peristalsis, hemodynamic instability and the addition of new or higher amounts of vasopressors
Feeding enterally during hemodynamic instability can lead to
gut ischemia and death
a concept relevant to critically ill patients who don’t tolerate nutrition, especially PN, intended to decrease complications of PN delivery by providing only 80% estimated energy needs until the patient’s condition improves is called
permissive underfeeding
this type of feeding is used in EN/PN for obesity patients to meet protein needs but provide less calories with BMI > 30 unless eight loss is not intended
hypocaloric feeding
what criteria should be met to start PN in the ICU
malnourished at baseline, not able to reliably ingest/absorb significant nutrition in 7-10 days, adequate resuscitation from any hemodynamic compromise, hemodynamically stable with paralytic ileus, acute GIB, or complete bowel obstruction, supplemental when EN doesn’t provide enough
