Test: Identify familiar odors Abnormal: Anosmia

Optic
Visual acuity- defect in absent central vision
Visual fields- defect in peripheral vision, hemianopsia
Shine light- absent light reflex
Direct inspection- papilledema

Oculomotor
inspection- abnormal= dilated pupil, ptosis, eye turns out and down
EOM- failure to move eye up, in, down
Shine light- absent light reflex
IV
trochlear. EOM failure to turn eye down or out
V
trigeminal.
superficial touch- absent touch, pain, paresthesias
corneal reflex- no blink
clench teeth- weakness of masseter or temporalis muscles
VI
Abducens- EOM muscles to right and left sides
Failure to move laterally, diplopia on lateral gaze
VII
Facial
Wrinkle forehead, close eyes tightly- absent or asymmetric
Smile, puff cheeks
3. identify tastes- loss of taste
VIII
Acoustic/Vestibucochlear
Hearing acuity- loss of hearing
IX`
Glossopharyngeal
Gag reflex- none
X
Vagus
Phonate ahhh- uvula deviates
Gag reflex- no gag reflex
Note voice quality- hoarse or brassy, nasal twang, husky
Swallowing- dysphagia, fluids regurgitate through nose
XI
Spinal Accessory
turn head, shrug shoulders- absent movement of sternomastoid or trapezius
Hypoglossal
Protrude tongue- deviates to side
Wiggle tongue from side to side- slowed rate of movement
