Olfactory:
Smell
Tested with various pungent odors (coffee, etc.)
CN II
Optic:
Visual Acuity: Confrontation method to assess visual fields
CN III
Oculomotor:
Eye movements: Follow the penlight through the six cardinal fields of gaze
CN IV
Trochlear: Eye movements (superior oblique): Same as oculomotor
CN V
Trigeminal:
Superfical and deep structures of head and face, mastication: Corneal reflex, show teeth.
CN VI
Abducens:
Specific eye movements: Testing same as oculomotor
CN VII
Facial:
Taste of anterior 2/3 of tongue, sensation in ear canal, facial movements: Use salt and sugar for taste, evaluate corneal reflex, raise eyebrows, keeps eyes closed against resistance, puff cheeks.
CN VIII
Acoustic:
Hearing: Ticking watch and whisper tests: Tuning fork with Rinne positive as normal when air conduction persists twice as long as bone conduction.
CN IX
Glossopharyngeal:
Swallow, taste the posterior 1/3 of the tongue, pain touch and temp of mucosa and pharnyx: Check for deviation of uvula to unaffected side.
CN X
Vagus:
Elevation of the uvula, swallow: Have patient say “ahh” and check uvula elevation. Check gag reflex and ability to swallow water.
CN XI
Spinal accessory:
Innervates the trapezius and sternocleidomastoid: Shrug shoulders and turn head against resistance.
CN XII
Hypoglossal:
Motor for the tongue: Push tongue against the inside of the cheek, stick out tongue and check for fasciculation and deviation.
