What is the tetralogy of Fallot?
A genetically transmitted malformation of the heart
What are the four malformations of the heart?
- Right Ventricular outflow obstruction
- Secondary Right ventricular hypertrophy
- Subaortic ventricular septal defect
- Overriding aorta (dextropositioned aorta)
What species are commonly affected by the tetralogy of Fallot?
Keeshonds and English Bulldogs
Cats
What are the clinical signs of the tetralogy of Fallot?
Stunted growth Exercise intolerance Collapse Cyanosis Sudden death
What does the heart murmur of a patient with tetralogy of Fallot sound like?
A precordial thrill may be felt in the area of the pulmonic valve
Murmur of pulmonic stenosis
Ventricular septal defect murmur
What do the radiographs of a patient with tetralogy of Fallot look like?
Normal-sized heart
Decrease in the pulmonary circulation
What does the echocardiogram of a patient with tetralogy of Fallot look like?
Right ventricular hypertrophy

What is the prognosis of patients with tetralogy of Fallot
Some animals can tolerate it for years
CHF rarely develops
Sudden death may occur
Genetically transmitted so do not breed
What is the treatment for patients with tetralogy of Fallot?
Conservative. Limit stress and exercise
Regular phlebotomy
Surgical correction
Why do patients with tetralogy of Fallot need regular phlebotomy
The PCV is increased to compensate for poor oxygenation
What is important for a vet tech to remember when treating patients with tetralogy of Fallot?
They may react adversely to sedatives, acquiring a slow heart rate that does not improve with O2 therapy
