What is the roll of the Coroner
Coroners are judges that sit on the Coroners Court (Equal to district court).
The coroner looks at evidence and determines:
- That someone has died
- The persons identity
- When and where the person died
- The cause of death
- The circumstances of death
What are the requirements to report sudden deaths to the coroner
Deaths that must be reported: S - Suicide C - Cause unknown U - Unnatural/Violent death M - Medical , surgical or dental operational B - Birth deaths or deaths related to pregnancy I - institutional E - Enactment requirement
What is the purpose of a post-Mortum examination? Who authorises and performs them?
Post Mortum is to examine a body and determine the cause of death
They are directed by the Coroner and performed by a pathologist
What is the purpose of a coronial inquiry and what form do they take?
The coronial inquiry includes the following establishments
- The fact somebody has died (Verification of death Cor31)
- The identity of the deceased (Deceased person identification form POL 265A)
- The cause of death (Pathologist report)
- Where and when the death occurred (POL 47)
- The circumstances of death (Statements, Job sheets)
What is an Inquest?
An Inquest is a formal hearing in the coronial court as a part of the wider inquiry
What are the circumstances in which an inquest must be held?
Coroner must hold an Inquest into the following deaths:
- Self Inflicted
- Any death that occurs in official custody or car
- Any death in which the coroner is not satisfied that the cause or circumstance has clearly been established
Define “Missing Person”
A person who is reported missing to police
AND
Who whereabouts are unknown
AND
There are fears for the safety or concerns of the welfare of that person
What is the procedure for taking a missing person complaint
Establish weather there are fears for a persons safety, if so, Why?
Create a missing persons report
What are three points I must consider before deciding on what action to take in relation to a missing persons report?
1) The age of a person
2) Zones of safety (Are they in their usual surroundings)
3) Circumstances I.e dangerous or suspicious
What to do when a missing person has been located?
Following steps to be followed:
- Interview thw located person about the disappearance
- If a child, look for abuse indicators
- Enter details of location and reason for going missing NIA
- Remove the alert from NIA
- advise informant that the person has been found
- update the missing person report
What are the contents of a missing persons file?
- File cover sheet
- Police report (Pol 258)
- Missing persons report (Pol 62 part A)
- Job sheets/Statements
- File reviews (Pol 62, Part B)
- Computer printouts
What are the stages involved with completing a Part B, 14 Day review of the Pol 62 Form
- Review the files update occurrence barrier I’ve and scan Pol Part B into occurrence
- Discuss case with O/C CIB
