the three checks
- regardless of distribution system, always check the drug label against the MAR three times:
1. before removing the medication from the shelf/cart
2. Before preparing or pouring the medication
3. Before replacing the medication on the shelf/cart (just before administering the medication)
order for giving medication
- hand hygiene
- prepare meds
- hand hygiene
- identify client
- client teaching
- administer medication
- sign MAR
- observe reception/reactions
- dispose of garbage appropriately
- hand hygiene
- document observations
eye drops/eye ointment
tilt head and put in lower lid pocket
ear drops
- without touching the skin surface, hold bottle tip or dropper above ear canal and have client tilt head toward unaffected side.
- infants/children under 3 years: pull pinna down and back
- adults and children over 3 years: pull pinna up and back to straighten ear canal
- client should remain side-lying 5 min, gentle pressure to tragus
nasal spray
- client in high fowler position
- client tilts head back
- place tip of the bottle just inside the nares, aimed toward midline of nose
- squeeze bottle for correct dose while patient inhales
- ask them to maintain position for 1-2 mins
transdermal meds and creams and ointments
- good idea to use gloves, but not policy
- only use black/blue ball point pen for documenting on a patch (write date, write month a word, time and initial)
inhalent medication
need to prime it if it hasn’t been used in a while or new. some you need to shake (hold top). 30 sec-1min before another puff, 5 mins if getting another medication
