CIMT Flashcards
What are the potential causes of hemiparesia
Causes - injury to one side of brain: CP, TBI, Hemisphecterectomy (removal of hemisphere), tumors, CVA
what are the signs and symptoms of hemiparesia?
Signs/Symptoms: on one side of body (oppposite side of brain injury)
weakness
Hypertonia in trunk and hypotonia in limb
Sensory impairment
Decrease use of affected limb=> becomes weaker
What is CIMT?
Restraint of uninvolved UE during intensive structure therapeutic activities (ex: if R side involved => constrain L side) => force-used of involved side
What are the benefits of CIMT
increase f of use of involved limb
Improves bilateral activities
Improves quality of use in functional activities
Cortical reorganization (increase cortical representation of affected side thus new connections)
what are the different types of constraint used?
permanent cast (=> if child who is younger or has limited safety, won’t put cast bc wont have reaction if fall)
Sling: less constraining and can maintain leisure activity
Orthosis
Glove (to prevent grasp)
Use touch or verbal to constrain
what are the characteristics of cimt group therapy (5)
(better bc more motivated)
- no significant behavioural difficulties or intellectual deficits
- motivated to participate in group
- GMFCS: level I or II (ambulant no walker)
- MACS: level I, II or III
- Homogenous age group w realistic functional goals
what are the characteristic of cimt individual therapy (4)
2-16 yo
family motivation
child’s collaboration
Realistic goals
Group CIMT Therapy:
- duration
- age:
- therapy type:
- type of constraint
50-60 hr/12 day camp constraint wear: 4.5 h/d age: 6-13 yr therapy: interdisciplinary types of constraint: homemade sling
Individual CIMT Therapy:
- duration
- age:
- therapy type:
- type of constraint
50-60 hr/2-10 wk
constraint wear: variable
age: 2-16 yr
therapy: OT 1X/wk, other professional if needed
types of constraint: verbal, physical, splint sling
what are the 3 key component of cimt
- Intensity & repetition: repeat movement
- motor learning approach
- graded activities to ensure just right challenge
What are some GM, FM and bilateral activities that can be done during CIMT
GM: yoga (increases flexibility, Obstacle course (balance), Unravel mummy (gross motor, coorindation, strength, proprioception), hydrotherapy
FM: painting (extension wrist/elbow), search in sand
bilateral: cutting food, tie shoes (often at end of day, have children work on functional task by removing sling to work bilaterally)
