Chronic Pancreatitis Flashcards
What is chronic pancreatitis
Continuing inflammatory disease of the pancreas characterised by irreversible glandular destruction due to chronic inflammation
What is the main cause of chronic pancreatitis
Alcohol abuse
What are the causes of chronic pancreatitis
Alcohol abuse
Cystic fibrosis
Malnourishment
Pancreatic duct obstruction
What happens to the pancreas in chronic pancreatitis
The pancreatic ducts are dilated and filled with protein plugs which can be calcified
Acinar tissue is destroyed
What happens to nutrient absorption in chronic pancreatitis
Fat Malabsorption as lipases are not present
What are the signs and symptoms of chronic pancreatitis
Pain in the epigastric region that radiates to back and improves when sitting forward Weight loss Stearorrhea Jaundice Secondary diabetes
What investigations should be carried out for the diagnosis of chronic pancreatitis
CT scan
MRCP
What is MRCP
Magnetic resonance cholangiopancreatography
MRCP produces detailed images of your liver, gallbladder, bile ducts, pancreas and pancreatic duct
What investigation should be performed first in suspected chronic pancreatitis and why
CT scan
To inspect for any calcification
If you do a CT which shows calcification then what is the likely diagnosis
Chronic pancreatitis
If you do a CT which shows no calcification then what needs to happen
An MRCP
What is the treatment for chronic pancreatitis
Alcohol cessation Analgesia Insulin (if needed) Enzyme replacement therapy Removal of gall stones (SURGERY)
Why might insulin be given to someone with chronic pancreatitis
The damaged pancreas may no longer be producing insulin. The insulin would help the patient reduce their Hyperglycemia.
What are the complications of Chronic Pancreatitis (x5)
Pseudocysts Duct Obstruction Diabetes Mellitus (from decrease in endocrine functions) Steatorrhoea Nutrient Malabsorption (from decrease in exocrine function)
What is a good pneumonic to remember the causes of chronic pancreatitis
OA TIGER Obstruction Autoimmune Toxin Ideopathic Genetic Enviromental Recurring injury
