Chloramphenicol, polymyxin Flashcards
Chloramphenicol MoA
binds 50S subunit inhibits protein synth
Chloramphenicol spectrum and indications
The first really broad spectrum
Severe SEs limits its use currently to: Brain abscesses Mixed resistant infections Meningitis Ricketsioisis in children
Chloramphinicol SEs
Gray baby syndrome
inadequate glucuronidation/elimination of the drug causes accumulation, interferes with mitochondrial human ribosomes.
Respiratory depression, hypoxia death
Dose dependent aplastic anemia
Hemolytic anemia in G6PDH def.
Interactions: Inhibits CYP. Increasing Warfarin and phenytoin concentrations.
Polymyxin - what are the drugs
A MIX of POLYpeptides
Polymyxin B -
Polymyxin E - colistin
Polymyxin MoA
binds membrane polypeptides creating pores
Polymyxin spectrum and indications
Against most G-neg bact.
Uses: Against P. aeruginosa or other significant pulmonary infections. Esp in Cystic fibrosis.
Polymyxin SEs
