Shadowing in the lungs is due to:
1. Alveolar disease
2. Interstitial disease

The main features of alveolar shadowing
A
- In the early stages it may present as faint homogeneous haze
- Shadowing maybe homogeneous and patchy in appearance.
- Air bronchograms maybe visible

A
Transudate
Exudate
Haemorrhage
Infiltration

Alveoli become filled with transudate of low protein content and occurs in pulmonary oedema.

Alveoli become filled with fluid of high protein content.
Caused by Adult respiratory distress syndrome

Alveoli filled with blood and this may occur in trauma

Patterns of alveolar shadowing
A
- Lobar/Segmental pattern
Caused by: infection, pulmonary infarct, contusion, alveolar cell carcinoma - Bat’s wing pattern
Acute: pulmonary oedema
Chronic: Lymphoma/leukemia, pulmonary alveolar proteinosis, alveolar cell carcinoma
