Root Cause Analysis
Retrospective
This is a retrospective investigation of an even that has already occurred, which includes reviewing the sequence of events that led to the error.

This is the goal for most health care settings to improve efficiency, quality, and patient satisfaction while reducing costs.
The Joint Comission
Is an independent not-for-profit organization that accredits and certifies more than 20,000 healthcare organizations and programs in the U.S
National Patient Safety Goals
