Chapter 74: Aminoglycosides: Bacteriostatic Inhibitors of Protein Synthesis Flashcards
aminoglycosides
Gentamicin,
Neomycin,
Amikacin,
Tobramycin
Only IV, Tobramycin can be aerosolized
Streptomycin,
GNATS
amino uses
Most commonly used for serious infections caused by gram negative rods (including pseudomonas, Klebsiella, Serratia, E coli).
Development of resistance is uncommon (especially with amikacin)
Needs concentration monitoring
Use limited by relatively serious toxicities (Ototoxicity nephrotoxicity). Hearing loss can be irreversible.
Poor lung penetration
AG therapuetic goal
Treatment of serious infections caused by gram negative bacteria.
AG baseline data
Blood and or urine cultures
AG monitoring
Aminoglycoside levels (peak and troughs) and renal function must be monitored.
AG identifying high risk pt
Use cautiously in renal impaired patient, pre existing hearing impairment, and those receiving ototoxic and nephrotoxic drugs
AG evaluating therapeutic effects
Monitor for reduction in bacterial symptoms: fever edema, pain, inflammation
AG min adverse effects
Use cautiously in renal impairment. Patient must report hearing loss
