Consumer health informatics
Field devoted to informatics from multiple consumer or patient views, includes patientfocused informatics, health literacy and consumer education, with a focus on information structures and processes that empower consumers to manage their own health
Continuity of care document (CCD)
The result of ASTM’s Continuity of Care Record standard content being represented and mapped into the HL7’s Clinical Document Architecture specifications to enable transmission of referral information between providers, also frequently adopted for personal health records
Continuity of care record (CCR)
Is a core data set of the most relevant administrative, demographic, and clinical information about a patient’s healthcare, covering one or more healthcare encounters. It provides a means for one healthcare practitioner, system, or setting to aggregate all of the pertinent data about a patient and forward it to another practitioner, system, or setting to support the continuity of care.
Health literacy
The ability to understand instructions on prescriptions drug bottles, appointments cards, medical education brochures, doctor’s directions, and consent forms. It also includes the ability to navigate complex health care systems It requires a complex group of reading, listening, analytical, and decision-making skills and the ability to apply these skills to health situations.
Internet forum
Web application for holding discussions and posting user generated content, also commonly referred to as web forums, newsgroups, message boards, discussion boards, bulletin boards or simply a forum
Learning health system
An ecosystem where all stakeholders can securely, effectively, and efficiently contribute, share, and analyze data and create new knowledge that can be consumed by a wide variety of electronic health information systems to support effective decision making leading to improved health outcomes
mHealth
The use of mobile cellular devices and other wireless personal devices focused on health and fitness monitoring
Patient activation measure (PAM)
Developed as a way to be able to predict the patient’s level of engagement in healthcare. It is 13-item Likert survey instrument that scores patients on scale of 1-100 and classifies patients as falling into one of four categories based upon their total score
Patient-centered care
Relationship-based primary care that meets the individual patient and family’s needs, preferences, and priorities
Patient-centered medical home (PCMH)
A program to provide comprehensive primary care that partners physicians with the patient and their family to allow better access to healthcare and improved outcomes
Patient engagement
Actions individuals must take to obtain the greatest benefit from the health care services available to them
Patient-generated health data
Health-related data created, recorded, or gathered by or from patients (or family members or other caregivers) to help address a health concern
Patient portal
Information system that allows patient to log in to obtain information, register, and perform other functions. A secure online website that gives patients convenient 24-hour access to personal health information from anywhere within an internet connection
Personal health record (PHR)
An electronic or paper health record maintained and updated by an individual for himself or herself
A tool that individuals can use to collect, track, and share past and current information about their health or the health of someone in their care
Personalized medicine
The tailoring of medical treatment to the individual characteristics, needs, and preferences of a patient during all stages of care, including prevention, diagnosis, treatment, and follow-up
