A

1) Fly-over-symbol enclosed in a circle, turn anticipation is NOT allowed. Missed approach and holding points usually are these
2) Fly-by-just the symbol, turn anticipation is allowed.

What is radius-to-fix and can USAF aircraft fly them?

A

RF and it is constant radius turn for initial and intermediate approach segments and missed approach, SID or STAR. Looks like arcing between GPS points and denoted in plate notes as RF required.
Usually no but depends on MAJCOM

1) procedure in DOD FLIP
2) Host nation on DOD accepted host nation list and procedure published in English
3) Procedure has current Foreign Terminal Instrument Procedure (FTIP) review
