Explain what is meant by “objective” and “subjective” information in the narrative portion of the prehospital care report. Explain what is meant by “a pertinent negative”
Subjective: is from an individual point of view “I FEEL dizzy”
Objective: observable, measurable, and verifiable “vitals”
Pertinent negative: is not present but important to note “The patient states that her chest pain does not radiate”
List some important steps to take and information to include when documenting a patient refusal.
- Try to persuade the patient to go to the hospital
- Ensure the patient is able to make rational decisions
- Inform patient why they should go and what could happen
- Consult medical direction as directed by local protocol
- Have patient sign form
- Have bystandard, family, police sign as witness
