hyperparathyroidism: recall the differences between primary, secondary and tertiary hyperparathyroidism

primary hyperparathyroidism
A
adenoma in thyroid gland so increase in PTH and increase in [Ca2+] but feedback cycle doesn’t work as adenoma

secondary hyperparathyroidism
A
low plasma [Ca2+] e.g. renal failure so increase in PTH but can’t restore Ca2+ levels

tertiary hyperparathyroidism
A
initial chronic low plasma [Ca2+] causing high PTH to increase [Ca2+]; patient then becomes cured but now parathyroids have become autonomous → release of PTH causes high [Ca2+] but no negative feedback to regulate
