Dihydropuridine (DHP) Drugs
Amlodipine, Nifedipine, Nicardipine, Felodipine, Isradipine, Nimodipine, Nisoldipine
DHP MOA
Blocks Ca channel on vascular smooth muscle/arteries which results in vasodilation: reduce BP
DHP Effects
selective, vasodilate arteries, minimal cardiac effect, not an antiarrhythmic
DHP ADRs
Reflex Tachycardia, peripheral edema, flushing, palpitations, hypotension, dizziness, fatigue
DHP therapeutic uses
Hypertension treatment, vasodilation, reduce BP
Precautions for DHP
Risk of exacerbating angina, precipitating arrhythmias
Non DHP: Verapamil
non selective, vasodilates, decreases HR and BP
Non DHP ADR
Constipation, dizziness, drug induced arrythmias, AV block, sinus bradycardia, ventricular fibrillation
Non DHP therapeutic uses
used for angina, increases oxygen supply and decreases oxygen demand, class IV antiarrhythmic
Non DHP: Diltiazem
non selective, vasodilation, decreases BP and HR, less constipation, flushing, rhinitis
Precaution with Non DHP
do not use on patients with Congestive HF or with beta blockers, or those with preexisting av block, pulmonary edema, and precipitating arrhythmias
