life-threatening blistering disease

pathophysiology of pemphigus vulgaris
IgG4 Abs against desmosomal proteins cause loss of keratinocyte adhesion in the skin and mucous membranes leading to superficial blistering, erosions, ulcers
histology of pemphigus vulgaris
superficial split!- separation of individual keratinocytes
common areas of PV
scalp, face, axilla, groin
s/s of PV?
oral ulceration, non-itchy flaccid blisters- fluid filled, blisters may rupture leaving erythematous weeping erosions
ix for PV?
biopsy & direct immunofluorescence of perilesional skin, Nikolsky’s sign
what is Nikolsky’s sign
skin rubs off easily
mx of PV?
systemic steroids, tetracycline, emollients etc
immunosuppression in long disease
prognosis of PV?
chronic self-limiting course, remission achieved between 3-6months
epi of bullous pemphigoid
more common than pemphigus, affects >60s
pathophysiology of BP?
deeper split! local compliment activation (auto IgGs) against hemidesmosomal protein causing split through DEJ
s/s of BP?
large tense serous/ haemorrhagic blisters, itchy
no mucosal involvement
