Up to how many hydrants can be reported on 1 CD-63?

Up to 7 defective hydrants may be reported on each form. If a unit has more than 7 defective hydrants, additional forms shall be used.
Information entered on the Form CD-63 can either be typed or hand printed. When printing, it is important that what color pen is used?

Hydrants shall be reported to DEP as defective when:

A. An inspection reveals a defect in a hydrant that has not previously been reported:
B. An inspection reveals that a hydrant previously reported as defective has not been repaired since the last semi-annual inspection period. Resubmit
the CD-63 with a notation entered in the remarks/defects column stating, “Previously Reported on mm/ dd/ yy.”
C. Upon receipt of a completed work ticket from DEP, a reinspection of the repaired hydrant reveals the repair work to be unsatisfactory
Exception: Do not resubmit a CD-63 for “Cap or Chain Missing” unless the defect has existed for __ months since the original CD-63 was submitted.

What is the procedure if a unit with an operational fax machine cannot connect with DEP’s fax machine?

How shall we forward the CD-63 when the units fax machine is OOS?

In the event that a unit’s fax machine is out of service, the form shall be forwarded to the
Administrative Battalion with a note requesting the Battalion to fax the report.

After fax transmission to DEP the CD-63 shall be filed in a folder, labeled CD-63, in front of the “Defective Hydrant” file.
Upon receipt of a DEP repair ticket, the officer shall remove the related hydrant card from the “Defective Hydrant” file and cause an inspection to be made. If the hydrant is found satisfactory, the hydrant disc shall be removed and an entry made, in ___, on the hydrant card, the repair ticket, and the CD-63. The notation in ___will state: “REPAIRED”, “DATE”, and “OFFICER’S NAME” who verified the repair.

The Hydrant Inspection Card shall be returned to its proper place in the file. Attach the repair ticket to the related CD-63. When all hydrants on the CD-63 are repaired the form shall be filed chronologically by repair date order in a folder called “Repaired Hydrants”. (Place this folder behind the hydrant card file). Repaired tickets and CD-63s must be kept for how many years from the date of the verified repair?

High pressure hydrants are no longer a viable source of water supply for firefighting and
should not be used. DEP will be removing these hydrants as funds become available. When should we inspect these hydrants?

Units shall not inspect these hydrants nor report them as defective.
High Pressure Hydrant Cards shall be removed to an inactive file and maintained until units are notified by DEP that the related hydrant has been removed. The hydrant card can then be discarded.
What notation shall be made when dealing with hydrants with a Custodian Locking Device?

Units shall note on Fire Hydrant Inspection Cards any hydrant that is equipped with a Custodian Locking Device by marking “CN” on the front of the card underneath the three digit repair I.D. number.
The presence of a Custodian Locking Device on a defective hydrant shall be noted by marking “CN” under the HYDRANT TYPE CODE heading on the CD-63.
A Priority Hydrant is:

A. a hydrant that is the only hydrant in a block; or
B. a hydrant(s) which is (are) vital to the protection of high profile locations or critical infrastructure locations e.g., bridges, tunnels, mass transit systems, etc.
How do we mark a priority hydrant on the BF-47A?
7.3
Marking “PRIORITY HYDRANT” in red on the top of the BF-47A shall identify priority hydrants.
Companies shall adhere to the following criteria in designating hydrants that require priority repair. Only hydrants satisfying one of these criteria shall be identified to DEP as requiring priority repair:
9.2.1, 9.2.2 & 9.3
-“Priority Hydrants” which are Out-of-Service.
-Two adjacent hydrants in a block that are both Out-of-Service (report both hydrants as requiring priority repair).
-The priority repair designation for such hydrants shall be noted on the CD-63 by marking
“PR” in the column under the “HYDRANT TYPE CODE” heading.
