Asthma: Cellular Flashcards
Asthma is characterized by variable airflow ___, airway ___ to histamines, and chronic airway ___
Asthma is characterized by variable airflow obstruction, airway hyperresponsiveness to histamines, and chronic airway inflammation
How would asma affect this volume curve?
FVC will lower, and FEV will lower. Therefore, the ratio will be low 70%, indicating an obstructive airflow disease.
It should revese with a drug or removal of trigger
severe asthma
during a methylcholine challenge test, if the FEV1 drops, we know that the person has:
airway hyperresponsiveness
2 types of noneosinophilic asthma phenotypes
- paucigranulocytic
- neutrophilic
2 types of eosinophilic asthma– which one is eligible for biolgics treatment?
- eosinophilic– may indicate inadequate corticosteroid therapy, but its what’s considered “Typical Asthma”
- mixed granulocytic. – eligible for biologics treatment
NOTE: SABA on demand was a first line therapy prn. But now it’s an ICS/LABA on demand.
note
note: eosinophilic asthma can be treated with steroids, and severe cases with biolgics/ IL inhibitors. paucigranulocytic asthma is more steroid resistance, with some affect with LABAs. Bronchial thermoplasty might need to be considered.
how do corticosteroids affect inflammatory cells
reduces eosinohphils, reduces cytokines from t cells, reduces number of mast cells, reduces cytokines from macrophages, and reduces dendritic cells.
