Optic nerve - Test with confrontation test
Inspect ocular funds (color, size shape)
III, IV, VI
Oculomotor, Trochlear and Abducens Direct and indirect consensual light and accommidaton cardinal positions of gaze presence of nystagmus? pendular movement? Amplitude Frequency? Plane of movement
V
Trigeminal
Motor - Palpate temporal/masseter muscles as the person clenches teeth
sensory - light touch with cotton 1. ophthalmic, 2. maxillary, 3. mandibular
Normal - no pain, symmetric, full sensation
VII
Facial nerve
Motor - have the person smile, frown, close eyes and puff cheeks.
note mobility and symmetry
VIII
Acoustic
whispered voice test
IX and X
Glossopharyngeal and Vagus
depress tongue
gag reflex
normal: symmetrical
XI
Spinal Accessory Nerve
rotate head against resistance
shrug against resistance
Normal: full strength, no atrophy strength equal bilateraly
XII
Hypoglossal Nerve
inspect tongue
no tremors or wasting
