What are the 3 major hypotheses for depression?
Neurotrophic, monoamine and steroids hypothesis
What is the neurotrophic hypothesis?
Brain-derived neurotrophic factor (BDNF) is thought to be neuroprotective and decreased in depressed pts
What is the monoamine hypothesis?
NE, serotonin, dopamine, and glutamate all play a role in depression
What is the steroids hypothesis?
Glucocorticoids, thyroid hormones and sex homrones play a role
What are the 5 R’s?
Response, remission, recovery, relapse and recurrence
What is response?
> 50% reduction in sx from baseline; not well just better; partial response = >25% reduction but <50% reduction in sx from baseline (less than full response)
What is remission?
Symptom free (very low to no sx); not only better, but well; a healthy state of functioning
What is recovery?
6-12 months of ongoing remission; not cured
What is relapse?
Return of sx after remission but before recovery
What is recurrence?
Reutrn of sx after recovery
What is Milnacipran?
An SNRI used only for fibromyalgia
What is Fluvoxamine?
An SSRI only for OCD
