Anesthesia/medicine Flashcards
What is a met?
basal oxygen consumption of a 40 yr 70kg male. shows functional status.
How does pulse ox work?
Measured the red and infrared and the ratio of the light that makes it through and the hgb concentration.
What are two disorders that can cause false elevation in oxygen concentration?
Carboxyhemaglobin(smoking)
methoglobinemia/ usually readings wont fall below 85 %
How do you treat methemoglobinemia??
Methylene Blue
Which leads are more sensative to cardiac ischemia ?
Leads II and V5.
modified aldrete score?
BP +- 20 of original
Oxygen sats above 92%
able to move 4 extremities on command(thumbs up)
Deep and normal breathing patter
Fully awake is 2 points arousable is 1.
Needs a 9 out of 10 for discharge.
pharmacodynamics(dynamic)
how the drug affects the body
what is a MAC?
Minimum alveolar concentration to prevent purposeful movement in 50% of patients. Usually make of 1.2 needed for GA in hospital settings.
Higher the mac the less potent the medication
CO2 scrubers and be KOH or NAOH.
Amsorb is better because it doesnt produce compound A.
Propofol MOA
induction does
Sedation dose continous and bolus
Potentiates GABA.
2 mg/kg.
50 mcg/kg/min or 20-50 mg boluses
ketamine MOA
induction dose IV and IM
Sedation .2-.5 mg /kg
NDMA recepter antagonist.
IV 1-2 mg. IM 3-5
Midazolam MOA
Sedation dose IV
What does flumazenil do?>
Potentiates gaba / Liver metabolism.
0.05-.1 mg/kg
Competitively binds to receptor. Dose is 0.2 mg iv over 15 seconds.
repeat every minute to a total dose of 1mg.
dose of Fentanyl? adult and child
dose of reversal? adult and child
1-2 mcg/kg for a child
0.4 mg/kg up to 2 mg for children its 0.01 mg/kg then try 0.1mg/kg.
Succinylcholine dose
1mg/kg for induction
.2mg/kg rescue dose for laryngospasm
What is the pediatric risk of Succinylcholine?
Rhabdomyolyses and kyperkalemia in undiagnosed muscular diseases.
