- Dyspnea/Hypoxia
- Wheezing/Stridor
- Difficulty Swallowing
- Oropharyngeal/Tongue Swelling
- Urticaria
- Abdominal Pain/Vomiting
- Shock/decreased perfusion

A non-anaphylactic allergic reaction is defined as ONE of the following..
A
- Localized Symptoms
- Localized Angioedema w/out Airway or GI symptoms
- Urticaria alone

Anaphylaxis is defined as..
A
Severe, acute onset & one of the following:
- Respiratory Compromise (dyspnea, wheeze, stridor, hypoxemia)
- Decreased BP (SBP<90)
OR
A combination of 2 of the following:
- Urticaria
- Swollen tongue or lips
- Nausea/Vomiting
- Abdominal pain
- Syncope
- Incontinence

If Localized Allergic Reaction..
A
Diphenhydramine 1 mg/kg
IV/IM (50 mg MAX dose)

If Anaphylaxis/Multisystem Reaction/Shock..
A
- 1:1 IM Epi 0.01mg/kg (0.3 MAX dose)
*May Repeat q 5-15 min - Cardiac Monitor & EtCO2
- Consider Airway Management
- Assess for signs of Shock (poor perfusion, decreased mental status)
- Breathing TX
- IV Access
- Fluid Bolus 20mL/kg (Hypotension)
- Diphenhydramine 1 mg/kg IV/IM (50mg)
- Solumedrol 2mg/kg (125mg MAX)
Transport, even if symptoms Resolve
