Anaphylaxis is defined as..

A

Severe, acute onset & one of the following:
- Respiratory Compromise (dyspnea, wheeze, stridor, hypoxemia)
- Decreased BP (SBP<90)
OR
A combination of 2 of the following:
- Urticaria
- Swollen tongue or lips
- Nausea/Vomiting
- Abdominal pain
- Syncope
- Incontinence

A

Diphenhydramine 1 mg/kg
IV/IM (50 mg MAX dose)

If Anaphylaxis/Multisystem Reaction/Shock..

A
  • 1:1 IM Epi 0.01mg/kg (0.3 MAX dose)
    *May Repeat q 5-15 min
  • Cardiac Monitor & EtCO2
  • Consider Airway Management
  • Assess for signs of Shock (poor perfusion, decreased mental status)
  • Breathing TX
  • IV Access
  • Fluid Bolus 20mL/kg (Hypotension)
  • Diphenhydramine 1 mg/kg IV/IM (50mg)
  • Solumedrol 2mg/kg (125mg MAX)

Transport, even if symptoms Resolve
