Alpha & Beta (adrenergic) blocker.

Indication
A
Rapid ventricular rate in atrial arrhythmias:
- refractory PSVT
- atrial tach
- atrial fib
VF/ pulseless VT
Stable VT.

Dose/Route
A
- Stable VT: 150mg over 10 min MR q 10 min followed by 1mg/min infusion.
- VF / Pulseless VT: 300mg rapid IVP MR 150mg in 3-5 min.

Hypersensitivity to the drug
2nd and 3rd degree heart blocks
Bradycardia

Adverse effects
A
CV= bradycardia, hypotension, negative inotropy Resp= pulmonary toxicity (pneumonitis, alveolitis) Skin= photosensitivity

Precautions
A
1) monitor EKG and vital signs
2) Slow or stop infusion if significant hypotension or bradycardia occurs
3) Ca channel blockers and beta blockers may potentiate sinus bradycardia, sinus arrest or AV block
