  4. Push an additional 20 mg (1 mL) lidocaine over 60 seconds;
  5. Connect a 10 mL NS preload to the extension set and push 1 mL over 60 seconds (thereby pushing in the 20 mg licocaine remaining in the extension set), then flush the rest of the NS preload rapidly;
  6. Reconnect the lidocaine syringe to the extension set and fill it with the remaining 20 mg (1 mL);
  7. Attach a NS flush to extension set and push 1 mL over 60 seconds (thereby pushin in the last 20 mg lidocaine remaining in the extension set);

Note: For pediatrics, use 0.5 mg/kg (max dose 60 mg), divide into three parts, and use a similar delivery technique;
