Precocious Puberty ages
before 8 years in girls
before 9 years in boys
Intraductal Pappilloma
benign slow growing tumor behind areola
bloody, discharge
Pysiologic Leukorrhea
normal increase in vaginal secretions 6-12 months prior to onest of menses
Primary Amenorrhea =
menses absent at 15
Secondary Amenorrhea =
absent for 3 months if regular cycles, 6 months if irregular cycles
Testing for Amenorrhea
Pregnancy Test LH, FSH Prolactin, TSH, free t4 total and free testosterone, DHEAS
Progesterone Challenge
give 10mg of medroxyprogesterone for 7-10 days
Expect bleeding 2-7 days afterwards, correlating with a progesterone deficiency
If progesterone challenge yields no bleeding…
either have disrupted anatomy or not enough estrogen
Give estrogen x 25 days + progesterone x 7-10 d
Abnormal Uterine Bleding Described as
cycles <20 days or >45 days
lasts >8 days
>6 full pads or tampons/day or anemia
Benign Pubertal Gynecomastia
~13 years of age
usually tender and asymmetric
normal resolves in 1 year
Spermatoceles
nodule above and posterior to the testes
sometimes transilluminates
Torsion of the testicular appendage
tender pea sized swelling at the upper pole of the testis w/ a blue dot sign
