the extent to which a person’s behavior corresponds with agreed recommendations from a health care provider =
what is the difference between compliance, adherence and concordance?
compliance = extent to which patient’s behaviour coincides with the clinical prescription
adherence = extentio to which behavior coincides with healthcare professionals agreed upon recommendations
concordance = extent to which the patient and the healthcare provider agree on the nature of the condiiton and need for treatment
that are the three essential elements of adherence?
- initiation = when the first dose is taken
- implementation - the extent to which the dose taken corresponds to the prescribed dose
- persistence= the length of time between initiation and discontinuation
what are the 5 main causes of non-adherence?
- condition related factors - chronic or acute? severe?
- therapy related factors - longer duration/frequent changes?
- health system related factors - poorly developed services?
- social/economic factors - illiteracy? unemployment?
- patient related factors - age, gender, social class
what are the two types of non-adherence?
unintentional - forgetting etc
intentional - choosing not to adhere
what beliefs/cognitions can contribute to non-adherence?
beliefs about condition - causes, timeline/duration, consequences (see common sense model of illness)
beliefs about treament - not believing treatments worth it
beliefs about necessity?- weigh pros and cons of treatment
what are the 3 stategies to maximise adherence?
- understanding - of the condition and its treatment
- memory - for information given
- satisfaction - with the process of treatment
