What is acne vulgaris a disease of?
PSU of the face, neck and back
What is a blocked follicle called?
comedone
Why are follicles blocked in acne vulgaris?
sticky keratinocytes and increased sebum viscosity
Why is there inflammation in acne vulgaris?
change in commensal bacteria behaviour
Hypertrophic scars
usually get better in 3-5 years without treatment
Keloid scars
outwith original damage and gradually get worse
Who does acne vulgaris affect?
15-18 year old
M=F but males get more severe
family history
Topical treatment for acne vulgaris
anti-biotics
retinoids
benzoyl peroxidase
Non topical treatment of acne vulgaris
isotretinoin/roaccutane
antibiotics
anti androgens eg OCP
light based treatment
Most effective acne treatment
roaccutane
Outcomes of roaccutane
60-70% cure and will expect clearance
major problem with roaccutane/isotretinoin
highly teratogenic
Other side effects of roaccutane
dry skin
hair loss, depression, abnormal LFT
Acne rosacea appearance
ace of clubs distribution on face
PSU, chronic inflammation and cutaneous vasculature
