A31. Drugs used in coagulation disorders III: Fibrinolytic drugs. Drugs used in bleeding disorders Flashcards
List the drugs used in bleeding disorders.
- Local antihemorrhagic
- Systemic
- Vitamin K
- Fibrinolytic inhibitors: omega aminocaproic acid derivatives (EACA), aprotinin
- Plasma Fractions
- Desmopressin
What is the MOA of desmopressin?
Desmopressin increases activity of factor VIII.
What are the indications for fibrinolytic drug use?
- Multiple PE
- Central DVT
- AMI
What are the indications of Desmopressin?
Can be used in pts with mild hemophilia A or vWF disease.
What are the indications of Vitamin K therapy?
Used for reversal of anticoag effect.
List the local antihemorrhagics.
- Vasoconstrictors: Epinephrine, NE
- Protein denaturing compounds: close small capillaries.
- Potassium aluminum sulfate: old aftershave.
- Large molecules with large surface: collagen, gelatin, fibrin, activate intrinsic pathway
What are the adverse effects of EACA?
- Intravascular Thrombi
- Hypotension
- Myopathy
- GI symptoms: Abdominal discomfort, Diarrhea.
- Nasal stuffiness
What are the adverse effects of taking Fibrinolytics?
- Bleeding: (incidence 15%, lethal 0.5%) short therapy → less bleeding complications
- Hypersensitivity rxn: in streptokinase, anistreplase, staphylokinase.
What are the Contraindications of taking fibrinolytics?
- Active bleeding
- Serious GI or cerebral bleeding w/in 3 months
- Surgery (incl biopsy), severe trauma w/in 10 days
- Severe hypertension
- Aortic dissection
- Acute pericarditis
What are the adverse effects of Aprotinin?
- Anaphylactic rxn
- Reversible transaminase and serum creatinine level ↑
What are the indications of Plasma fractions?
Plasma fractions are cryopreciptates from whole blood. It is given for deficiencies in plasma coag factors.
What is the route of administration and DOA of Vitamin K?
Vitamin K can be given orally or IV (IV can be problematic, can cause dyspnea, chest and back pain, possibly death). The onset of effect delayed by at least 6 hours, complete effect is not seen until 1 day has passed. Routine admin for all newborns, esp. premature.
What are the indications of EACA?
- Bleeding from fibrinolytics
- Post-surgical bleeding
- Adjunct therapy in hemophilia
- Prophylaxis for rebleeding from intracranial aneurysms
What is the MOA of omega aminocaproic acid derivatives (EACA)?
EACA are Competitive antagonist at lysin binding sites of plasminogen. Structurally EACA are similar to lysin.
What is the MOA of Aprotinin?
Aprotinin is a serine protease inhibitor, isolated from cattle lung. It inhibits not only plasmin, but tryspin, kallikrein, chymotrypsin. No absorption from oral admin. parenteral use only.
