• Type 1 respiratory failure: pO2 < 8.0, normal or low pCO2
- Failure of oxygenation
• Type 2 respiratory failure: pO2 < 8.0, CO2 > 6.0

What is respiratory failure?
A failure in the respiratory system in one or both of it’s gas exchange
functions:
• OXYGENATION
•ELIMINATION OF CARBON DIOXIDE
Causes of Type 1 failure (hypoxia)
- Pneumonia
- COVID-19
- Pulmonary embolus
- Asthma
- Interstitial lung disease
- Pulmonary oedema
- ARDS/Acute lung injury
Causes of Tyoe 2 Resp failure
- COPD
- Neuromuscular disorders (Polio, MND, polymyositis)
- Obesity
- CNS depressants (benzos, opiates)
How do you asses Resp failure?
- ABCDE
- History and examination
- Cardio respiratory disease
- Smoking history
- Drugs (benzos, opiates)
What to look out for in the resp exam?
- Oxygen sats (? hypoxic)
- Respiratory rate
- Body habitus and work of breathing
- Alert?
- Bounding pulse? sign of CO2 retention
- Use of accessory muscles
- Flap? sign of CO2 retention
- Listen to the chest
- Air entry?
- Wheeze?
- Crackles?
