Criteria for diagnosis of ADHD?
Several observations before age 12
seen in multiple settings
clear evidence that symptoms interfere with/reduce quality of life
not due to psychosis, depression or anxiety
Types of ADHD/ specifiers
Inattentive
Hyper/Impulsive
Mixed
Also specify mild, moderate, or severe
How many symptoms do patients need and how long do they have to have them to be diagnosed with ADHD?
at least 6 symptoms, persisting at least 6 months
Is ADHD heritable?
Yes, but no exact gene is knonwn
Common comorbidities with ADHD?
Oppositional defiant disorder (seen in 50%)
Conduct disorders (25%)
Tests to diagnose?
Formal= CPT - continuous performance task
Practical= ask about video games, movies
Personalities commonly associated with ADHD?
Increased risk taking
decreased harm avoidance
reward dependence
Treatment for stage 0 ADHD only?
Adjunctive therapies, special education if associated with learning disabilities
Dietary treatment/holistic strategies
Treatment for stage 1 & 2 ADHD only?
Pharmacotherapy= psychostimulants
Methylphenidate
Dextroamphetamine
Amphetamine
Dexmethylphenidate
Treatment for stages 2 & 3 ADHD only?
Treat with non-stimulants = atomoxeitine
NonFDA= atypical antidepressants like buprorion, venlafaxine, duloxetine
Treatment for stages 4 & 5 ADHD only?
Treat with tricyclic anti-depressants
Desipramine, imimpramine, amitriptyline, mirtazapine
Treatment for stage 6 ADHD only?
Treat with alpha 2 agonist- blood pressure agents used to calm hyperactivity, impulsivity, little focus
Treatment for ADHD with depression?
Treat whichever disorder is more severe first then ad treatment for second disorder if monotherapy does no result in remission of both disorders
Treatment for ADHD with anxiety?
Atomoxetine
