6: Integrative Oncology Flashcards
What are some common types of mind-body modalities?
Energy based - reiki
manipulative/body-based - chiropractic care, massage, acupuncture, yoga
alternative medical systems - Ayurveda, traditional Chinese medicine
Natural products/supplements - herbs
Define CAM & IM
Complementary and Alternative Medicine - Complementary is used along with mainstream and Alternative is uses instead of mainstream medicine
Integrative medicine - more recently used, emphasizes the integration of conventional practices with those that have been considered CAM and supported by evidence of safety and effectiveness
NIH for Complementary and Integrative Health classifies into what 2 categories?
Biological Products - dietary supplements, special diets - most commonly used complementary approaches among patients/survivors
Mind-body interventions (MBIs)- deep breathing, qigong mediation, tai chi, and yoga
What group of patients (age/sex/education/diagnosis) are more likely to use CAM?
Breast cancer patients are the most prevalent (partly due to research) but so are GYN
20-30 year olds, over 60 year olds females
more prevalent among widowed, retired, and highly educated women
What are some motivational factors that could influence a patient to pursue CAM? What tends to be the underlining factor?
influence - social relationships, cultural, spiritual, and religious beliefs, practitioners, family/friends, folks in the waiting area/support groups
Underlining factor - provides patients with a way to be more involved in their own care – they are doing something vs something being done to them - help manage vulnerability, loss and uncertainty feelings
What is one major factor MBIs (mind/body interventions) try to address?
STRESS
higher stress = increased inflammation, increased depression, decreased immune function
What percentage of all adult cancer patients have used CAM in the last year? What cancer type has it even more commonly?
35.3% for all
Breast Cancer – 43.6%
What is the most common forms of CAM?
Vitamin/Mineral supplementation - 78%
Herbal supplementation - 27.9%
What are the most common forms of MBIs?
Deep Breathing - 11.6%
Yoga - 9.5%
Mediation - 7.6%
Massage Therapy - 6.9%
How common was Acupuncture at NCI Centers? What is its proposed effects?
73%
Often used to help with QOL. managing N/V from chemo, and postop symptoms. Given a Grade B for N/V in breast, but a Grade C for anxiety/stress/hot flashes
What are some proposed benefits from Qigong and Tai Chi?
Significant benefits have been shown for both with improving fatigue, sleeping difficulty, depression and QOL.
Also found to have some improvements with post op upper-limb lymphedema, lung function, sleep quality, limb muscle strength, fatigue and distress
What are some proposed benefits from Meditation?
Reduction in depression, anxiety, stress and improved QOL
One study did find a possible impact on cytokine levels (increasing interleukin-4 and decreasing interferon-y) and aiding the immune system
How many NCI centers had yoga? What’s the most common type and how many types are there?
68.9% offered it at centers
there are 12 types — Hatha yoga is the most common, which focuses on postures (asanas) and breathing exercises (pranayama)
What are some of the proposed benefits from yoga?
Increase heart rate variability, a measure of ANS activity that can be low in stress-related imbalances
increase release of dopamine, neurotransmitter y-aminobutyic acid, and lower salivary cortisol levels
Improve fatigue, sleep quality, depression, anxiety, and QOL
Given a Grade B for anxiety, mood/depression
How could yoga play a larger role in improving survivorship?
Yoga practice has been shown to reduce stress, improve body satisfaction, and improved mood. Been shown to have a positive impact on body weight, partly by being associated with healthier eating and exercise. All improve body composition, which could reduce adipose tissue/cancer risk
