Define a Hazard
A workplace condition that might result in injury, health impairment, illness, disease or death to any worker exposed to the condition, or might result in damage to or loss of property of equipment.
Hazard Severity
Assessment of the worst potential consequence is likely to occur due to deficiencies.
What is an Injury
Traumatic wound or other condition of the body caused by external force including stress or strain.
Mishap Probability
The likelihood a hazard will result in a mishap or loss.

Web-Enabled Safety System
- A web-based safety mishap data collection and reporting system developed for the Navy and Marine Corps by Commander, Naval Safety Center (COMNAVSAFECEN).

Mishap Investigation and Reporting Purposes
- Identify Hazards with potential to cause frequent and severe mishaps
- Could cause future mishaps
- Identify unsafe acts and conditions, corrective measures
- Identify mishap casual factors and develop appropriate corrective actions, will help prevent future mishaps
Investigation and Reporting Required Conditions
Any other work-related illness or injury that involves medical treatment beyond first aid.
- Loss of Consciousness
- Lost Work Day
- Light Duty or LIMDU
Gov. Motor Vehicle OR Gov. Vehicle Other Mishaps Resulting in what?
$5,000 or more gov. vehicle, gov. property damage, POV pr private property damage.
More Investigation and Reporting Required Conditions
- Any Medically diagnosed work-related illness and injury
- Work Related STS
(a) Averaging in 10 dB or more at 2000, 3000 and 4000 Hz in one or both ears.
-Any work-related need stick injury or cut from a sharp object that is contaminated with another person’s blood or infectious material.
Mishap Classifications
Class A
- Damages $2 Million or more, injury to occupational illness resulting in fatality or permanent disability
Class B
- More than $500,000 but less than $2 million, when three or more are hospitalized for inpatient care as a result of a single mishap
Class C
- More than $50,000, less than $500,000, nonfatal injury or illness resulting in 1 or more days away from work, not including the day of injury
Class D
- $20,000 or more, less than $50,000. Recordable injury or illness not classified as A, B or C.
Accident and Injury Reports
Medical Department will submit A&I as directed to the CO

Safety Investigation Report
- Submitted to COMNAVSAFECEN
- Using WESS
- Using WESS vis email
HAZREP
Hazard Report
- Submitted for near-mishaps and submissions where lessons learned should be shared.
