300-10 Transitional Duty

What forms need to be inter offices mailed within 24 hours of the BFOE being completed

1) Original BFOE indicating accepted or denied
2) original dwc form 73 or treating physician statement

FMLA Correspondence accepting or declining coverage.

How long is transitional duty originally approved for?

Answer 2

90 calendar days.

*Employee must submit a new DWC form 73 every 30 days or after every visit **

Question 3

Both civilian and classified employees may request extensions up to how many days?

Answer 3

90 extra days for a total of 180 days. In increments up to 90 days.

*must be in writing sent you through chain of command up to assistant Cheif level * must be sent 14 calendar days before transitional days are to begin.

Commanders shall send written referrals to Administrative personnel committee (APC) if:
