Emergency Operations
The Administrative Assistant Bureau Commander for the geographic area where the accident occurred, the on-duty 24-hour platoon duty Assistant Chief, the Risk Management Section (RMS) on-call Safety Officer and the Los Angeles City Attorney shall be notified on all accidents involving LAFD apparatus that are serious in nature, i.e., fatality, serious injury, etc. Requests for the City Attorney to respond to the incident can be made through Metropolitan Fire Communications (MFC) or via the RMS Safety Officer.
Chief Officers shall identify any witnesses and ensure that civilian witnesses complete a

Examples of time sensitive issues include but are not limited to:
significant traffic accidents, in-custody deaths, wrongful deaths, firefighter fatalities/serious injuries, high potential for litigation

An accident in which fire Department personnel exercised reasonable judgment and precautions, exhibited good apparatus/vehicle operator’s and Officer’s skills and observed all State, local and fire department statutes, ordinances and rules.
Category I
An accident which occurred because of inattention to vehicle operator’s and/or Officer’s responsibilities or because of misjudgment of clearance in operating apparatus near stationary objects.
Category II
An accident which occurred because of an apparatus/vehicle operator’s misjudgment of clearance from other vehicles, deficiency in defensive driving techniques, or failure to anticipate other vehicle’s movements.
Category III
An accident in which fire Department personnel failed to follow established Department rules and regulations, policies and procedures. PSD will handle accountability to disciplinary guidelines for any member that is found to be at fault, the accident rises to the level of discipline, and discipline is recommended for such by the Traffic Accident Hearing Officer. The PSD disciplinary guidelines currently address driving violations. They can be accessed on the CTS site in the Help/References Section.
