When conducting the AAR, the discussion should capture the following:
- Incident Overview (Type of Incident, Building Description, Situation, Scenario)
- What was planned? (Objectives, Strategy, Risk Assessment)
- What actually happened? (Communication, Deployment, Tactics, Organization)
- Why did it happen? (Challenges, Successes, Failures, Lessons Learned)
- What can we do next time to improve? (Correct Weaknesses, Sustain Strengths, Improve Performance)
The leader should facilitate the discussion by identifying resources based on their sequence of arrival in ______ order or sequence of events in a training exercise.

Formal AAR’s will consist of ____ level review or higher AAR accompanied by an After Action Review Report (AARR). The deployment of a Serious Incident Review Team (SIRT) to construct a Blue Sheet and Green Sheet or Final Report, is also considered formal and will require specific timelines for completion.

Any all-hazard incident reaching the level of a Major Emergency or Multi-Operational Periods, and/or consisting of unique challenges or hazards, safety concerns, equipment failure, deployment issues, lack of command and control, or need for analysis by Emergency Operations shall be considered _____.
Except in unusual circumstances, these types of incidents will require a directive from the Bureau Commander, Deputy Department Commander (DDC), or Chief Deputy of Emergency Operations to conduct a formal AAR no later than __ hours after the incident or training exercise and require an AARR immediately following. The Blue Sheet is only a Preliminary Summary Report and may be followed by a more comprehensive report (____ or Final Report). A disclaimer to that effect will be included on the cover of the document.

The Green Sheet is an Informational Summary Report utilized for review of a serious injury, near miss, and/or fatality and shall be normally prepared within thirty (__) calendar days of the SIRT’s arrival to the incident or training site unless otherwise directed.

what is a serious injury?
Inpatient hospitalization requiring admission. A loss of a body part. A serious, permanent disfigurement.
what is a near miss?
An event that occurs in the work or training environment and reasonably could have, but did not, result in a death or serious injury due to good fortune and/or proper operation of safety devices or equipment.
The Fire Chief, Emergency Operations Chief Deputy and Administrative Operations Chief Deputy − in consultation with the Deputy Department Commander and the United Firefighters of Los Angeles City, President − will initiate the Review Team from the SIRT Cadre to prepare an initial Blue Sheet followed by a Green Sheet

The ___ is an investigative body that will be charged with confidentially assembling an unbiased factual report based on the collection of evidence and interview of witnesses following a serious injury, near miss, and/or fatality.
SIRT (serious incident review team)
The SIRT Final Report shall be required for a serious injury, near miss, and/or fatality when the investigative process will exceed __ days to compile a comprehensive report. This report does not have a time limit for completion due to the complexities of the investigation and necessary collaboration with other investigative bodies. Either a Blue Sheet or Green Sheet or both shall precede the Final Report. In addition to the Final Report, a confidential Working Recommendations Report will be assembled for the Fire Chief by a separate body, briefed by the SIRT, and include causal and contributory factors, recommendations, and/or commendations.

Any incident or training event requiring a formal AARR shall have all associated records/documents maintained in the corresponding Bureau Office for a minimum retention of five (_) years. These files shall be archived in the “R” Drive under a “Bureau After Action Review” folder.
