Member requesting

1. Prepare LOA report and submit to commanding officer/supervisory head for approval, at least ______ days before leave commences except in emergency.

A. Uniformed and civilian MOS must complete the location during absence section on the LOA report including each country, with city/town or province where the member will be staying, date of departure and date of return unless on military leave or leave of absence without pay (30) calendar days or more.

B. If member will remain at their residence member will enter residence

C. Supervisory head will ensure the location part of the LOA report is filled out.

3. Member who is granted extended leave of absence without pay must take all accrued leave prior to the start of leave of absence, except for military leave.
4. Leave without pay for (30) or more consecutive days during a year, except military leave, will reduce authorized vacation by 1/12th for each 30 days consecutive days of absence.
5. Member returning from leave without pay for one year or more may not be granted unaccrued vacation until member performs active duty for a minimum of 3 months unless authorized by law.
6. An MOS uniform or civilian applying for any extended leave is required to communicate with the military and extended leave desk for instructions.
7. Leave without pay may be granted to observe a religious holiday. No more than 1/6th of each squad may be granted such leave.
8. If an emergency arises and MOS needs an Eday UMOS must get approval of who?
