Prepare a typed letterhead to deputy commissioner IAB containing following info

A. Date time location of fund raiser, the identities of the organizers and the recipient of the proceeds after expenses.

B. The results of inquiries made to intelligence bureau and detective bureau relative to the premises and the principals involved, and the state liquor authority when appropriate

C. The results of a check of the respective precincts integrity monitoring file

D. The specific manner funds will be raised ( ex go fund me), method of compensation to the principles premises, including an assurance that the full retail price will be paid for all food, beverages and ancillary services, such as music

E. A statement indicating that no illegal gambling, raffles or 50-50 drawings will take place at even

F. A statement and documentation indicating a specific accounting of funds collected and disbursed will be forwarded, writhing 30 days after the fundraiser to the deputy commissioner internal affairs. Copies of receipts etc.

G. A statement indicating that no conflict with dept procedures and or guidelines was found to exist relative to the fund raiser.

H. A concluding remark recommending approval of the request

2. Forward completed request through channels to deputy commissioner internal affairs
3. Advise CO submitting request of the IAB number assigned to the request and whether it was approved or disapproved by dep com iab
4. Return request if approved the bureau chief counterpart concerned for appropriate follow up and event monitoring
