2. Disorders of Sodium Balance Flashcards
What is pseudohyponatraemia?
Increased lipid or protein portion of blood, or high glucose
What is the lowest possible osmolality in the distal tubule?
50
What is the highest possible osmolality in the distal tubule?
1400
Define hyponatraemia
<135
Define anuria
Urine output less than 500 ml
Why are marathon runners in danger of water intoxication?
ADH is released due to high stress so urine osmolality is not suppressed and excrete an too little water
How does heart failure affect urine concentration?
Low CO results in decreased baroreceptor firing, causing increased ADH secretion
What are the signs of SIADH?
Reduced serum osmolality
Urine osmolality > plasma
Urine sodium reflects intake
What are the neurological causes of SIADH?
Meningitis
Tumour
Trauma
What are the pulmonary causes of SIADH?
Lung cancer
TB
What are the stress causes of SIADH?
Pain
Vomiting
Post-op
What are the drug causes of SIADH?
Ecstacy
Antipsychotics, SSRIs
Narcotics
Cyclophosphamide
What severe symptoms can be caused by hyponatraemia?
Water goes into cells: in the brain this causes cerebral oedema
What is the treatment for severe symptoms of hyponatraemia?
Hypertonic saline: 3% 150ml over 20 mins
What is the treatment for mild symptoms of hyponatraemia?
Normal saline (0.9%)
