2: Cerebrovascular Disease Flashcards
Four MC Cerebrovascular disorders
- Global ischemia
- Embolism
- Hypertensive intraparenchymal hemorrhage
- Ruptured aneurysm
When blood flow to the brain is reduced, survival depends on what three things
- Collateral circulation
- Duration of ischemia
- Magnitude and rapidity of flow reduction
Examples of global vs focal ischemia in the brain
- Global: cardiac arrest, shock, severe hypotension
2. Emboliform or thrombotic arterial occlusion, vasculitides, atherosclerosis in HTN
Shape of necrosis from a watershed infarct
Sickle-shaped band
Hemorrhagic conversion
Fragile vessels in an ischemic infarct rupture, leading to secondary hemorrhage
Ischemic stroke
Inadequate blood supply -> infarct of brain tissue
Binswanger Dz
large area of subcortical white matter undergoes myelin and axon loss
Charcot-Bouchard microaneurysms
Chronic HTN -> minute <300uM aneurysms in the basal ganglia
Three things to consider if a young pt has a stroke
Arterial dissection, PFO, hypercoagulability
